I always thought it was because - with a second set of water bottle mounts - there wouldn't be room for a Silca frame-fit pump by the seat tube.  Tsk, tsk.  Silly me!

Back in-the-day, when braze-ons were just coming back into vogue, we would go to the NY bike show and look at the new bikes from Europe and ask, why only one water bottle mount? The Italians would look at us and say, "Why would anyone want more than one? Are you Americans that crazy?"
