Just guessing here, but could it be a way to try and prevent the drive side BB cup from unscrewing? As we know, the drive side of English and many other BBs has a LH thread to keep it from unscrewing due to the bearing action. Italian all RH thread BBs can theoretically unscrew on the right RH side. If clamp isn't a Legnano feature, maybe it's something added by a previous owner.
