Clamping damage is always an "accident". Someone either clueless or caught off guard by a change in the setting or something. Sometimes someone will clamp a top tube (please, no!) not realizing the cable housing or even cable itself can make an impression. The only way I know to be safe is to never clamp a frame by the tubes to work on it (unless you are a painter or framebuilder). Even as a framebuilder, the easiest way to damage a tube is by clamping. You really have to be careful no matter how simple the task may seem. That's just my advice, do with it what you will.
