Actually, it's not impossible to set up a left hand crankset and I've seen it done in rare cases. You do forego using a freewheel or freehub, as the cog is of necessity a fixie. Putting the chainwheel on the left only requires a long enough spindle or a forgiving chainstay geometry, or some combination thereof.
