The Avocet Mod. III meets all the requirements.
It is a robust sealed tapered roller headset with alloy, somewhat traditional cups.
A little research and I found it pictured and described in US patent 4303257. If you go to the USPTO.gov website, download the alternatiff viewer, and you can do a patent number search to find this patent, then click on images to see the cutaway drawings etc.
This headset was fairly expensive, hard to find and is a bit tough to install because the lower cartridge has to fit into both the head tube and the fork. I had to make some tools and still chipped the paint on the bottom of the fork a bit. Also it requires dead correct faces on the fork and frame. I suspect a used one would never be worn out if you can find one.
