Here's my take on packing and since I build 'em I've thought a lot about it. I don't rely on the pipe insulation etc gambit. I'd rather keep the frame from being hit. I think Cannondale has the right idea. They strap the bike to a "pallet" inside the box so that it rides suspended from the sides. That's what I do. Whole bikes need an oversize Cannondale box for this-easily obtained from a Cannondale dealer-but a frame and fork will ride in a standard box quite easily. Just get 2 so that you can make up the pallet. Then I fill it with crumpled newspaper and Bob's your uncle. Works a treat.
