I recently snapped the spindle on a Olimpic 64 where it steps down in
diameter outboard of the inner bearing race.
No warnings, just snap, rider stayed up, foot hit the road attached to
the pedal, no major injury only a sore ankle for a day or two.
In defense of the pedal it was well used looking, of unknown heritage,
just period correct and had the misfortune of being strapped to to a
near 100 kilogram rider trying to rapidly accelerate his mass on a track
bike.
