[CR]1950's urago ten speed

I recently acquired this ten speed that I don't know much about and I could use some help with ID

Year, model of bike, model of crank, and size of original tires since the current sew-ups don't seem to fit exactly.

Also, I can only shift between the middle three of five cogs in back. The rear derailer is a simplex543 and is set to the "five" setting
