My understanding was that the covers were never meant to be used on the QR levers or DT shift levers, just for the bar-end shifters. Interestingly (to me at least) the bar-end shifters when first introduced (1951) didn't have the rubber covers. Must of been a nano second before someone said "HEY... these hurt my hands, hey?"
Yeah, I've seen photos of Merckx with the covers on his DT shifters and QR levers...
