I have a set of Gipiemme dropouts with a threaded, crescent shaped piece of metal to hold the adjuster bolt. They do in fact fall forward when the wheel is removed, but the knurled nut on the end prevents them from being lost.
FWIW, I also have a set of Simplex dropouts with a slightly different arrangement: a threaded plastic cylinder fits into a hole drilled through the dropout to hold the adjuster bolt.
