I think that Drew may have it, though it looks like they also turned a shoulder on the short side to get enough material to clear the holes from the turning off of the flange as it would have been too small of an area. It seems to have been slightly offset in the indexer, as the hole drilling was pretty imprecise and not concentric.
a fairly easy job with a mill and a lathe.
