On the old 4-speed freewheels, the outer cog was flush with the freewheel body and the extension on the tool allowed it to grip better without slipping. Rather than redesigning the tool when 5 and later 6-speed blocks became popular, Campy just made the silly insert to fit in the tool and allow access to the removal slots.
